Agent Commission Fee Form

      • Please complete the online form below after the second Friday (10th day of classes) of each academic term (September, January or May).  Commissions will be paid to registered companies (not to individuals) and only after the receipt of this completed online form. 

        Payment will be processed after the course add/drop period (3rd week of school) when the student's course schedule is finalized.All cheques will be issued in the company's name and sent to the address listed below.
